Change of Bank Account

If you wish to change your bank account, please complete the Application Form for Interbank GIRO and mail it back to us.

We wish to inform you that the bank normally takes 2 months to process the new Interbank GIRO application. In the meantime, please continue to send us your payment.

Change of Contact Details

Please contact our Customer Service Officers at (+65) 1800 880 4888 for assistance if you wish to change your postal address to a P.O Box / VBox.

Change of Personal Particulars

If you wish to update us on a change in personal particulars such as NRIC Number / Name / Residence / Citizenship, please complete the Request for Change Form and mail it back to us together with the following:

  • Change of NRIC Number — A photocopy of identity card (both sides).
  • Change of Name — A photocopy of identity card (both sides) and Deed Poll (for change name request only).
  • Change of Residence (e.g. Becoming a Singapore Permanent Resident) — A photocopy of the new identity card (both sides) and passport including the re-entry permit.
  • Change of Citizenship — A photocopy of both sides of the Citizenship Certificate and new identity card (both sides).
Change of Premium Payment Method

You may change your premium payment method by completing the following form and mail it back to us.

  • To change premium payment method to GIRO, please complete the Application Form for Interbank GIRO.
  • To change premium payment method to cash / cheque, please complete the Request for Change Form. Please note that the cash / cheque payment method is only available to non-monthly payment mode.

Change of Premium Payment Mode (Frequency)

If you wish to change the frequency of your premium payment, please complete the Request for Change Form and mail it back to us.

Please note that monthly premium payment is only accepted through Interbank GIRO, kindly complete the Application Form for Interbank GIRO and mail it back to us.

Change in Riders / Benefits

If you wish to make changes to the riders / benefits attaching to your policy, please contact your adviser or call our Customer Service Officers at (+65) 1800 880 4888

Change of Signature

If you wish to change your signature, please complete the Request for Change Form and mail it back to us. If you are unable to reproduce the old signature, please visit our Customer Service Centre personally with your identity card to make the changes.

Change of Sum Assured

If you wish to change the sum assured of your policy, please contact your adviser or call our Customer Service Officers at (+65) 1800 880 4888

Duplicate Policy Documents

If you have lost / misplaced / destroyed the policy document and require another copy, you are required to sign the Statutory Declaration as to Loss of Policy Form in the presence of the Commissioner of Oath.

Alternatively, you may visit our Customer Service Centre personally with your identity card to sign an indemnity form.

There will be an administrative charge of S$20.00 for issuance of a duplicate copy.

Policy Assignment

If you wish to assign your policy, both you and the assignee must visit our Customer Service Centre personally with your identity cards to execute the assignment. However, this is not required if the assignment is to a financial institution / company.

Premium Payment Options

You can choose to make payments to AXA Life Insurance Singapore Pte Ltd:

1) By Cash or NETS

At AXS Station
You can pay your premiums for Singapore dollar policies by cash card or NETS at any AXS station. You will need to key in the policy number and amount when making payment. Payment at any AXS station should be made before the expiry of the premium payment grace period.

2) By Cheque

You can mail your cheque together with the Premium Notice to us. Cheques should be crossed and made payable to AXA Life Insurance Singapore Pte Ltd. Please remember to write the policy number on the back of the cheque.

3) By GIRO

You can choose to arrange your premium due to be deducted from your designated bank account through GIRO. It is a convenient way to pay your premium. Just complete and return the Application Form for Interbank GIRO.

Please note that one application form is required for each policy and the bank normally takes about 2 months to process the new Interbank GIRO application. In the meantime, please continue to send us your premium payment.

Reinstatement of Policy

If you wish to reinstate your lapsed policy, please complete the Health Declaration Form and mail it back to us together with your payment. If you are unsure of the amount of premium and interest required to reinstate your policy, please contact your adviser or call our Customer Service Officers at (+65) 1800 880 4888

Surrender of Policy / Reversionary Bonus

If you wish to surrender/terminate your policy, please contact your adviser or call our Customer Service Officers at (+65) 1800 880 4888

Termination of Policy

If you wish to terminate your policy (with no cash surrender value), please complete the Request for Change Form and mail it back to us together with a photocopy of your identity card (both sides).
